Soldier Ensures Physician Completes DA Form 5888-1

Rules for Soldiers in Defense Attaché Program

Obtain a DA Form 5888-1, Screening of Family Members in Remote OCONUS Areas, and have the Family physician complete one form for each Family member. The Soldier or spouse may have to assist the physician in answering some questions; for example, the question on special education may require assistance. You can download a copy of DA Form 5888-1 from the Forms menu on the left.
