Soldiers located on or near an Army Post in the U.S. must follow the steps shown to complete the overseas screening process. Select each step for more information.

Soldier Completes Block 1-7 of DA Form 5888 MDP Completes Block 8 of DA From 5888 Soldier Completes DA Form 7246 Soldier Arranges Appointment for Screening at MTF Army MTF EMFP Practitioner Completes DA From 5888 If no medical or developmental problems are identified... If a family member requires further evaluation for possible enrollment in the EFMP... If the family member is a school-age child and there is a suspected educational need...
