Other Situations
There are other circumstances whereby a Family member may be dis-enrolled. These types of situations and the required documents for dis-enrollment are provided as follows:
- Incarceration of the Family member: requires a court order
- Family member placed in foster care: requires a court order. This situation is also subject to review by the Military Medical Treatment Facility and the Regional Health Command to determine if dis-enrollment is appropriate.
- Family member becomes a Ward of the State: requires court order indicating placement has occurred and when the placement occurred.
