Unregistering Family Members-Disenrolling Soldiers
 

Appropriate medical or educational personnel must complete DD Form 2792 and/or DD Form 2792-1 to verify that the condition has improved (or no longer exists). Take the completed forms to the EFMP office in your local Army Military Treatment Facility (MTF). The EFMP office reviews the information and forwards the forms to an Army Regional Health Command (RHC). If the Family member no longer meets the medical or educational criteria, the RHC unregisters the Family member and disenrolls the Soldier. This process may take several weeks.
