If a Family member is enrolled in EFMP prior to screening, the medical practitioner indicates whether there has been a substantial change in severity of the condition and related medical needs since enrollment. If there has been a substantial change (or if the last update is almost 36 months ago), a new DD Form 2792 and DD Form 2792-1 (if needed) is completed. A new DD Form 2792-1 is also completed if the Exceptional Family Member Special Education/Early Intervention Summary is older than 1 year at time of screening. The updated information is attached to the DA Form 5888.
