If a Family member requires further evaluation for possible registration in the EFMP, the Family member is asked to make an appointment with his/her primary care manager (or other appropriate specialist) for completion of DD Form 2792, Exceptional Family Member Medical Summary.
Upon completion of the DD Form 2792 (if needed), the EFMP practitioner checks consideration for enrollment warranted in block 9B of DA Form 5888. If warranted, a copy of the EFMP enrollment materials are provided to the Family to take to the Soldier's Unit Personnel Office.
