Soldiers Located on/or Near Army Post in U.S.

If no medical or developmental problems are identified, the physician or medical practitioner indicates enrollment not warranted in block 9a of DA Form 5888. The Form will be return to the Family member to take to the Soldier’s Unit Personnel Office for Family travel processing.
