Soldier Completes Blocks 1-7 of DA Form 5888                                                                       

The Soldier should obtain a copy of DA Form 5888, Family Member Deployment Screening Sheet, and complete blocks 1-7 in consultation with his or her Personnel Division.  All Family members who are to accompany the Soldier to the overseas location must be listed in box 7.

Special note: 1. Soldiers serving in the attaché system who are requesting a consecutive overseas assignment must have their Family members screened prior to approval of accompanied Family travel. 2. Screening is also required when the Soldier requests an extension of a 36-month tour for any period longer than an additional 6 months.
