Soldier Completes DA Form 7246
Soldier should obtain a DA Form 7246, Exceptional Family Member Program Screening Questionnaire. It is suggested that the Soldier and spouse complete the form as directed. This questionnaire is about the health and education history of the Family members and helps the EFMP staff conduct the review process. Family members listed on the DA Form 7246 should coincide with those listed on the DA Form 5888.
