Family Member No Longer Eligible for Care
A Family member may no longer be eligible for care if they have reached the age of 21, or no longer meet the college or incapacity guidelines in DODI 1000.13, Identification (ID) Cards for Members of the Uniformed Services, Their Dependents, and Other Eligible Individuals.
Family member must submit a printout or approved DD Form 1172, Application for Uniformed Services Identification Card DEERS Enrollment, obtained from the local ID Card Section to verify that the Family member is no longer DEERS eligible
