If the Family member is a school-age child and there is a suspected educational need, the Family is given a copy of DD Form 2792-1, Exceptional Family Member Special Education/Early Intervention Summary, to be completed by the child’s school or early intervention coordinator. During the summer months, the EFMP office assists with the completion of the form. If the child is receiving early intervention services or special education, a copy of the Individualized Family Service Plan (IFSP) or an Individualized Education Program (IEP) must be attached to the form.
 

Upon completion of the DD Form 2792-1 (if needed), the EFMP practitioner checks consideration for enrollment warranted in block 9B of DA Form 5888. If warranted, a copy of the EFMP enrollment materials are provided to the Family to take to the Soldier’s Unit Personnel Office.
