Soldier Makes Appointment for Screening                                                                               

The Soldier contacts the local military Medical Treatment Facility (MTF) EFMP office for an appointment for the Family members to complete the EFMP screening packet.

 

Special Note: 1. The DA Form 5888 is good for one year from the date of authentication in Part B, block 11e. 2. If the assignment selection process or training extends beyond 1 year, a new DA Form 5888 and screening are required before accompanied Family travel can be approved. 3. DA Form 5888 must be dated within 1 year of the Family's projected arrival on station. If not, a new DA Form 5888 and screening are required. 4. The Soldier must provide information to an Army EFMP MTF Office if the medical or educational condition of any Family member changes.
